CareVacations will pay a set commission fee for ALL special needs equipment reserved, provided Travel Agent completes the Client Order Form in advance, faxes or emails it in to CareVacations and order is confirmed, paid and successfully completed. CareVacations will provide confirmation of the order to the Travel Agent with a copy to Client if requested.

Commission check will be issued approximately 30 days after completion of travel. Commission is based on 10% for all confirmed/traveled Mobility orders, and a flat commission fee of $20 for each confirmed/traveled Oxygen Order (min $200. order). Commission amount subject to change without notice.**Recall of commission may be required if client cancels or is refunded in any manner. **

Travel Agent is responsible to ensure that information transferred to CareVacations in regard to Agent/Agency is accurate and amended as required. Agent/Agency can be withdrawn from the program at any time.
